🎯 ROLE OVERVIEW

As a Senior Population Health Strategist, you will play a key role in developing targeted interventions to address chronic disease management for our health plan members. You will be part of a dynamic team that uses data-driven insights to identify areas of focus and develop strategies to address specific health disparities. You will work closely with our clinical team to develop and implement programs that improve health outcomes for our members. You will also collaborate with external partners to leverage their expertise and resources to drive population health initiatives.

📋 KEY RESPONSIBILITIES

Develop and implement population health strategies to address chronic disease management, including developing targeted interventions and programs to improve health outcomes for our members.

Collaborate with our clinical team to design and deliver high-quality, patient-centered care that meets the needs of our diverse member population.

Conduct data analysis to identify areas of focus and develop strategies to address specific health disparities.

Work with external partners to leverage their expertise and resources to drive population health initiatives.

Develop and maintain relationships with key stakeholders, including health care providers, community organizations, and government agencies.

Assist with grant writing and reporting to secure funding for population health initiatives.

Collaborate with our data analytics team to develop predictive models to identify high-risk populations and develop targeted interventions.

Stay up-to-date with the latest research and best practices in population health and share knowledge with the team to drive innovation and improvement.

✅ REQUIRED QUALIFICATIONS

Bachelor’s degree in a related field, such as public health, health education, or health administration.

At least 8 years of experience in population health, health care management, or a related field.

Experience with data analysis and reporting, including proficiency in statistical software and data visualization tools.

Strong understanding of health care policy and regulatory environment.

Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders.

⭐ PREFERRED QUALIFICATIONS

Master’s degree in public health, health administration, or a related field.

Experience with grant writing and reporting.

Certification in a related field, such as Certified in Public Health (CPH) or Certified Health Education Specialist (CHES).

Fluency in multiple languages, with ability to communicate effectively with diverse populations.
